Clinical Team Coordinator/Manager
Maria Parham Home Health is seeking a Clinical Team Coordinator/Manager. The position is responsible for the oversight of a clinical, multi-disciplinary team providing direct patient care services in the home. Responsible for initial referrals on clients and the coordination of patient care activities of the home care team. Communicates with clients, physicians, referral sources and others to promote quality care and customer satisfaction. Ensures compliance with standards of care and adherence to all federal, state, and local laws, accreditation, NC Board of Nursing, and Maria Parham Medical Center policies. Supervises the home care clinical team.
